Here’s a timeline of all the known warnings, incidents and reports about the Lyell McEwin Hospital short stay unit, which SA Health kept open for almost three years despite those warnings.
We know that safety reports from the unit’s staff and correspondence from the doctors’ and nurses’ unions were sent to SA Health.
Principal Community Visitor Maurice Corcoran says the reports from his office were routinely sent to the department.
